How Do I Get My Records Transferred?

The process is pretty easy. 

1. Due to HIPAA, you have to sign a form to release your information from your old clinic. We have releases below for MCHP and for other providers. Please fill in your name, date of birth and then sign & date at the bottom. You can only do one form per patient

2. Take this form to your old clinic and turn it in

3. Your old clinic will fax us your records.
